Clark takes Mary captive in Backstage and ties her to a chair. There, he tells her he has made peace with "Pirate Clark," the embodiment of his own destructive nature, and can now live in peace in this dimension.

Mary challenges Clark's faulty reasoning, and he decides to hug the pirate Clark to show her she's wrong. But when he hugs his insane double, he bites and kills him.

Parsons told Polygon that he hopes fans will see this moment as "the engine of the film," as Clark's growing obsession stems from how the Underworld seems to fill an emotional void in his life. The pirate's bite on Clark could be a manifestation of Clark's deepest emotional wounds and desires—or perhaps the monster simply needed to feed.

«"It's entirely possible that Clark was just particularly hungry that day, and there's no specific reason for [Pirate Clark's attack]," the director explained to Polygon. "I don't think that's the case from a story standpoint. But there's no way to know for sure because we don't hear a monologue from Pirate Clark.".

Before Pirate Clark kills Clark, he frees Mary from her chair. After the attack, she begins to flee, and the monster continues to pursue her until she reaches what appears to be a furniture store.

The therapist knocks out Pirate Clark with a cement handprint she stole from Backstage and is captured by the Asyncron Research Institute, a company that monitors the other dimension. Viewers see Mary being interrogated by Asyncron employee Phil (Mark Duplass), as well as a twisted version of Mary sitting alone and silent.

These disturbing moments leave it unclear whether the Mary who escaped from Backstage is the real therapist or a distorted copy created by this dimension.

«"[Sequels] aren't just an option, it's been planned since 2022," the director told Polygon in May 2026. He began making YouTube videos about Behind the Scenes four years before the release of his debut film, inspired by an anonymous photo of an empty yellow room posted on 4chan.

The eerie image, first posted in 2019, spawned a viral internet folklore known as "Behind the Scenes"—a creepypasta concept that describes an unsettling void one can enter by "stepping outside of reality," like a glitch in a video game. Parsons explained to USA Today that he was struck by how many people empathized with "this thing they can't quite articulate," and he developed a larger plot that extends far beyond a single film.

«"This film is the first installment of what I'd like to see happen in several story stages, in terms of getting closer to what I consider the true essence of the idea," the director later told Polygon. While A24 hasn't confirmed whether a sequel is in the works, a film series would be a "dream scenario" for Parsons.

«"And overall, I don't think the series is defined by genre labels," he added. "I see it more as an interpersonal drama built on the foundation of a supernatural techno-thriller. It's the genre I feel most comfortable in.".
